What are the responsibilities and job description for the HUD Rapid Rehousing Intake Coordinator position at Economic Opportunity Council of Suffolk Inc?
MAJOR RESPONSIBILITIES:
- Spanish speaking preferred.
The Intake Coordinator for the HUD Rapid Re-Housing Program is the first contact between Consumer and available HUD Rapid Re-Housing services.
The Intake Coordinator will:
1) Determine the eligibility of those Consumers who seek assistance by completing the screening forms during the initial interview.
2) Process and prepare preliminary assessments of all low-income families referred to the HUD Rapid Re-Housing Program
3) Make appropriate referrals to all community partners and resource providers for crisis intervention
4) Complete prescreen/ intake assessment over the phone.
DETAILED RESPONSIBILITIES:
- Screen consumers for HUD Rapid Re-Housing Program entrance by phone interview and Face to Face.
- Complete the Eligibility Screening Form for consumers (a) confirming a low-income Consumer family's eligibility for COC HUD Rapid Re-Housing Program; and (b) documenting the eligibility of persons applying for homelessness prevention assistance
- Assist consumer with crisis intervention and short-term housing stabilization which includes aiding consumer with accessing emergency shelters/transitional housing, and supportive services to best /address the consumer immediate crisis needs.
- Refer consumer who do not qualify for the HUD Rapid Re-Housing Program to other mainstream services for assistance, or to other housing stabilization programs in their service area when they do not meet eligibility
- Track consumers who are screened but not eligible for the HUD Rapid Re-Housing
- Maintain an intake log of all those who request assistance and also complete an intake.
- Maintain Consumer intake screening forms.
- Complete monthly CAPTAIN reports to generate statistics of clients served in program.
- Communicate to potential participants; the process of the HUD Rapid Re-Housing Program and required documentation to proceed with program participation.
- Participate in all appropriate staff, supervision, and training meetings.
- Assumes essential duties for program success as assigned by the HUD Rapid Re-Housing Program Manager/ or designee.
REQUIRED QUALIFICATIONS:
- Excellent verbal and written communication skills
- Strong organizational skills and meticulous attention to detail
- Team Player with good listening skills
- Proficient in the use of Word, Excel and Power Point
- A car for visits to Consumers in their own homes or at other locations necessary to conduct intake and to deliver checks to landlords.
- REQUIRED Valid New York State Driver's License and own insured Vehicle.

EDUCATION/TRAINING/EXPERIENCE:
MINIMUM QUALIFICATIONS
High School Diploma or equivalent and a minimum of 1-year experience working with homeless population.
Must have knowledge of customer service skills; previous telephone system experience preferred.
Must be well organized, a self-starter, and able to work as a member of a team. Proficient computer skills.
