Position Summary
The Patient Care Coordinator is responsible for Program Planning, Human Resource Management, Financial Management, Quality Assurance, Staff Development, Collaboration with Medical Staff, Hospital Departments and external agencies.
Responsibilities
1. Accountability for Program Planning and Decision Making
Maintain Accountability for patient placement and bed utilization.
a. Initiates, monitors and/or participates in program development, implementation, evaluation and revisions.
Promotes public relations to increase customer awareness of services provided.
b. Acts on behalf of the Hospital administrative staff to coordinate hospital services by making decisions that positively impact patient care. Has the power to implement decisions that are made and is the final authority in the hospital during the absence of the unit managers and administration.
c. Provides written policies and procedures, keep job descriptions current, and maintain other important documentation.
d. Facilitate the implementation of the external/internal disaster plan and other emergency situations.
2. Human Resources Management
a. Counsels, supports, and mentor unit staff with decision making and strategizing how to best provide care to current and potential patients.
b. Coordinates with Resource, Charge and/or staff nurses or other health care providers in the supervising of staff.
c. Utilize workload data to justify and adjust staffing needs.
d. Adhere to legal and organizational requirements related to personnel management.
e. Initiates immediate disciplinary action as appropriate with proper documentation.
f. Demonstrates the knowledge and skills necessary to provide services appropriate to the age of the patient.
g. Participates in interview process for supplemental staffing, Assist Team and leadership as appropriate.
h. Participates in the evaluation of the performance of the Assist Team. Holds Assist Team accountable for performance within their job description.
i. Promote employee satisfaction. Directs the workforce to ensure safe, quality patient care by redistributing staff and relocating patients.
3. Financial Management
a. Utilize personnel in a cost effective manner.
b. Participates in the budget planning process.
c. Adheres to the overall hospital financial plan.
d. Work with departments to maintain within their budgets as staffing and resource decisions are made.
e. Identifies, encourages and report cost saving opportunities.
f. Secure and allocate human and material resources according to identified needs while maintaining accountability for cost containment.
g. Authorizes medication approval and/or transportation for indigent patients.
6. Staff Development
a. Actively participates on hospital and/or patient care services committees.
b. Attains and promotes professional growth and development through continuing education and nursing leadership activities.
c. Demonstrates the ability to work with others as a team member and leader. Assumes responsibility for personal clinical competence and continuing education including maintenance of professional credentials.
d. Demonstrates the cultural attributes with interaction of the customers.
7. Clinical Research
a. Demonstrates support of research mission of the organization through support of research activities in programs.
b. Identification of research topics
c. Assist in evaluation of the relevance of research efforts specifics to programs.
d. Assists staff in interpreting and implementing research findings in programs.
e. Keep current on research findings for various committee and program activities.
4. Quality Management
a. Creates an environment for empowerment and continuous quality improvement.
b. Serve as a change agent in organizational transformation.
c. Assesses, intervenes in and evaluates unit functioning by making rounds on patient care units.
d. Collaborates with department of legal affairs and risk management to prevent and/or decrease hospital liabilities.
e. Participates in quality activities thru monitoring of physicians and staff practices to meet the VMC mission, values and strategic plans.
5. Collaborates with all hospital departments, medical staff and external agencies.
a. Creates climate to maintain and facilitate open lines of communication for the promotion of effective working relationships.
b. Collaborates with Administrators on Call of bed status, patient/staffing issues and other pertinent issues daily and as appropriate.
c. Communicates with news media regarding official inquiries, ensuing patient confidentiality and positive public relations.
d. Collaborate with Physician Arbitrators and Facilitators, Patient Access Services, Unit Staff and regional referral agencies in patient placement and appropriate issues.
e. Communicates to Administration and notifies Managers of pertinent issues that have occurred related to departmental services or personnel.
f. Promote organ procurement through communication with Carolina Donor Services.
g. Provide leadership for ECU Health Donor Resource Team.
h. Collaborates with company police to ensue the safety of our customers, guest and employees.
i. Serves as consultant to all departments for problem solving and decision-making.
j. Provides Occupational Health Services to employees: including for cause screening, blood exposure process, infectious disease work restrictions, and appropriate health screening to return to work.
k. Functions as a liaison between Physicians, Patient Care Units, Patient Access Services, and outside referral facilities to coordinate admission of patients to, and transfer of, patients within the hospital.
Minimum Requirements
BSN or higher required
NC RN license required.
American Heart Association BLS or Heart Saver is required.
Five years of clinical experience, with at least one year of management or leadership experience is required.
Evidence of progressive leadership qualities is required.

Location: ECU Health Medical Center
Shift: Rotating 12hr Shifts
Hours: This is a PRN position with a minimum requirement of one(1) 12hr shift in a six(6) week period.
Weekends: Rotating
On-Call: No
