Position Description:

The Case Manager is responsible for identifying and maintaining resources that contribute to our clients' success. They develop care plans, assist clients with goal setting and accountability, and collaborate with clients as they progress through the various Eddy House programs.

Essential Job Functions may include the following or other duties as assigned:

• Serve as the primary advocate for clients and assist clients in achieving goals through appropriate referrals, transportation, advocacy, support, and other services
• Complete intakes and needs assessments
• Assist in client de-escalation and stabilization as required
• Participate in weekly staffing and case conferencing meetings
• Maintain timely and accurate case notes in our online databases
• Create client care plans and meet with clients regularly to check the progress of their goals
• Communicate regularly with clients, service providers, and other team members to ensure clients are on track to meet goals
• Coordinate services and referrals with community partners
• Build and maintain positive working relationships with community partners
• Provide education, presentations, and training to community partners and Eddy House staff
• Participate in the Emergency Phone rotation
• Maintain professional, client-focused, and helpful communication
• Assist others as requested or assigned, and perform related duties as required
• Maintain a safe and clean work environment
• Maintain professional and courteous appearance and demeanor
• Share in the ownership of Eddy House Values: Collaboration, Dignity, Grit, Kindness, and Hope

Requirements/Qualifications

• Required: Ability to pass a background check.
• Valid Nevada driver’s license and reliable vehicle with current vehicle registration and insurance.
• Minimum: Bachelor’s degree or higher in Social Work, Psychology, Criminal Justice, Public Health, Human Development and Family Studies or related field.
• Previous experience as a Case Manager for at-risk youth, preferred.
• Must demonstrate high level of confidentiality and professionalism.
• Proficient with Microsoft Suite and high degree of computer literacy.
• Strong interpersonal skills with the ability to communicate clearly and in a calm and pleasant manner.
• High level of organization and attention to detail with demonstrated interpersonal skills.
• Ability to read and write checklists, reports, and correspondence. Ability to read and comprehend complex instructions, memos, etc.
• Ability to analyze and perform complex computations with reasonable speed and accuracy. Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
• Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

While performing the duties of this job, the employee is regularly required to sit, use hands, stand, walk, bend, drive, and have specific vision abilities to include close and distance vision, and ability to adjust focus working with computer business equipment and while driving. Must be able to lift up to 25 pounds on occasion. The noise level in the work environment is usually low level.

The overall nature of the position is sedentary requiring little physical effort and occasional light exertion. There is occasional exposure to environmental conditions such as heat, cold, and temperature changes.
