- Certified Nurse Case Manager

- Hospice & Palliative Care Certification
- Certified IV Therapy Nurse (CRNI)
- Certified Rehabilitation Nurse
- Certified Wound Ostomy Continence Nurse (CWOCN)
- Certified Dementia Alzheimer's Disease and Dementia Care Trainer
Requirements:
- Valid New York State RN License
- 1 year nursing experience, preferably in acute care or in homecare
- Home care experience preferred but not required

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.”
