Job Title: PRN Physical Therapist Assistant
We are seeking a skilled Physical Therapist Assistant to provide rehabilitation services in our skilled nursing facility, Edenbrook of Oshkosh. This is a PRN position that offers the opportunity to work with a dedicated team and make a positive impact on the lives of our patients.

- Deliver quality therapy services, including treatment interventions, program implementation, discharge planning, and related documentation and communication under the supervision of a physical therapist.
- Prioritize and maintain an efficient work schedule that meets the needs of our facility.
- Collaborate with Therapy management staff to foster a positive working relationship with all Facility staff.
- Adhere to policies and procedures essential for daily rehab department operations.
- Evaluate patient needs for evaluation under the supervision of a PT.
- Treat patients to relieve pain and develop or restore function to achieve maximum performance under the supervision of a physical therapist.
- Monitor patient response to intervention, communicate changes, and follow up with updates to treatment modalities, goals, and frequency as needed.
- Complete documentation requirements in a timely manner.
- Communicate and collaborate with team members regarding client care.
- Assist with data collection on patient progress under supervising PT.
- Plan patient discharges when maximum benefits have been met in coordination with the supervising PT.
- Provide accurate billing summaries as required.
- Attend in-service education sessions provided to rehab staff.
- Maintain a neat and well-organized therapy department.
- Perform administrative duties to support therapy services.
- Identify new rehab admissions and notify the physical therapist when necessary.
- Communicate upcoming patient discharges with Facility staff as required.
- Participate in care plan meetings as requested.
- Provide in-service education to staff as requested.
- Supervise physical therapist assistant students as needed.
