Job Overview
We are seeking a compassionate and dedicated Registered Nurse - Home Health to provide high-quality care to patients in their homes. Responsibilities
- Conduct comprehensive assessments of patients’ health status and medical history.
- Develop and implement individualized care plans based on patient needs and preferences.
- Administer medications and treatments as prescribed, including IV infusions.
- Monitor patients’ progress and document medical records accurately.
- Collaborate with interdisciplinary teams to ensure coordinated care and effective discharge planning.
- Provide education and support to patients and their families regarding health management and disease prevention.
- Perform diagnostic evaluations and assist with trauma medicine as needed.
- Maintain compliance with health regulations and safety standards while providing care in the home setting.
- Travel to patients' homes as required, ensuring timely visits and continuity of care.
Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Previous experience in home health, nursing home, or assisted living settings is preferred.
- Strong knowledge of anatomy, medical records management, and phlebotomy techniques.
- Experience with discharge planning and trauma medicine is a plus.
- Ability to perform diagnostic evaluations effectively.
- Excellent interpersonal skills with the ability to build rapport with patients and families.
- Strong organizational skills and attention to detail in documentation and patient care.
- Willingness to travel for patient visits as needed.
