Location: Lowville, NY
Duration: 13 weeks
Schedule: 40 guaranteed hours per week (5x8-hour day shifts)

Job Summary:
We are seeking an experienced Home Health and Hospice RN for a 13-week contract in Lowville, NY. The ideal candidate will have experience providing compassionate, patient-centered care in a home health and hospice setting. This role requires knowledge of NDOC EMR and OASIS Medicare assessment.
Responsibilities:
- Provide skilled nursing care to patients in their homes, focusing on hospice and palliative care needs.
- Perform comprehensive patient assessments, develop care plans, and collaborate with interdisciplinary teams.
- Utilize NDOC EMR for documentation and complete OASIS assessments per Medicare guidelines.
- Educate patients and families on disease processes, symptom management, and end-of-life care.
- Administer medications, treatments, and interventions per physician orders.
- Participate in on-call rotation, responding to urgent patient needs.
Requirements:
✔ Active New York RN license (Compact licenses not accepted)
✔ Minimum 2 years of recent Home Health and/or Hospice RN experience
✔ Experience with NDOC EMR and OASIS (Medicare assessment) preferred
✔ BLS (Basic Life Support) certification (American Heart Association or equivalent)
✔ Valid COVID-19 vaccination card (No exemptions)
✔ Ability to work independently and manage a case load effectively
