LICENSED CLINICAL SOCIAL WORKER

The Licensed Clinical Social Worker (LCSW) will utilize advanced clinical social work practice in an outpatient setting. The LCSW will provide services to help minimize psychosocial stressors, assess, diagnose, and educate patients with mental health, prevention and treatment of psychosocial dysfunction, impairment, or disability, including mental, emotional, and behavioral disorders for individuals and groups. The primary role for the LCSW will be to conduct interviews and assessments, set treatment goals, and arrange coordinating services with the primary care team.

 

ESSENTIAL FUNCTIONS

  • Diagnoses and provides brief interventions and referring individuals to formal mental health, SUDs, and/or psychiatric program if clinically indicated with disorders.  Also addresses various behavioral and emotional disturbances.
  • Coordinates activities with treatment team that is in the best interest of the patient.
  • Documents timely and accurately in the patients Electronic Health Record (EHR) on all services provided.
  • Completes paperwork accurately and thoroughly using clear, concise documentation.
  • Assures patient charts are complete, legible, and up to date while maintaining at least a 90% or better quarterly chart review score.
  • Completes initial assessment of patients within three (3) working days of the referral for medical social work services.
  • Develops a patient care plan and assists patient throughout the plan.
  • Maintains a working relationship and involvement with other service organizations within the service area.
  • Participates in general department, treatment team, and case supervision meetings.
  • Develops documentation of patient discharge/transition plan when necessary.
  • Adheres to agency policies and office protocols (work hours, voicemail, staff meetings, etc.).
  • Provides consultation to medical providers regarding the behavioral and non-medical drivers of health components of a patient’s presentation. Develops collaborative plan to address symptoms and/or SDOH.
  • Maintains superior interpersonal skills necessary to deal effectively and courteously with patients, families, multi-disciplinary team members, department peers and community agency members.
  • Interacts with patients, providers, and staff in a professional, courteous, and responsible manner.
  • Participates in clinical staff peer review process, including quarterly chart audit.
  • Maintains confidentiality.
  • Performs other duties as directed by supervisor.

Qualifications


EDUCATION AND EXPERIENCE

  • Must have a master’s degree in social work from an accredited school
  • At least three years direct counseling experience in an outpatient setting and with underserved community members.

 

REQUIREMENTS

  • Complete annual education as required by Texas state licensure board requirements.
  • Qualified to provide crisis intervention as indicated
  • Must maintain a current and unrestricted Texas Social Worker LCSW License.

  • Able to use computer to chart patient information in the Electronic Health Record.
  • Ability to work productively and independently to meet timelines paying close attention to detail.
  • Able to follow the professional and occupational standards of the Texas State Board of Social Workers.
  • Ability to work well with community agencies, congregations, organizations, volunteers, and other resources.
  • Displays comfort visiting patients in low-income neighborhoods.
  • Demonstrates compassion for people struggling with homelessness, poverty, mental illness, and addiction.
  • Committed to a patient-centered and trauma informed approach to case management.
  • Supports current organizational initiatives and exhibits creativity and flexibility in times of change.
  • Promotes, models, mentors, and leads customer service excellence.
  • Communicates consistently with Chief Clinical Officer and other providers.
  • Must have the ability to work at least a 32-hour a week schedule with no physical limitations in normal ranges of sitting, standing, walking, talking, and lifting to 25 pounds.
  • Strong community building skills. Experience, ability, and willingness to communicate and work with staff, volunteers, and program participants from a variety of racial, cultural, lifestyle, and economic backgrounds.
  • Ability to conduct and participate in meetings at various locations in a professional manner.
  • Define problems, collect data, establish facts, and draw valid conclusions.
  • Able to communicate effectively in English and Spanish, both verbally and in writing.
  • Bicultural (English/Spanish) preferred.
  • Necessary technical skills to use electronic health records, incident reporting system, email, and other information systems.

Environmental/Working Conditions:  LCSW must work in provider area near PCP.  LCSW will need to adapt to various workspaces including exam rooms and administrative offices. 

 

Mental Demands: Mentally challenging position due to continuous exposure to extreme issues and mental health trauma. 

Salary : $75,000 - $110,000
