Job Responsibilities:
- Conduct comprehensive client assessments to determine needs, abilities, and preferences.
- Develop and implement personalized care plans to promote health, safety, and independence at home.
- Coordinate and arrange services and resources to support clients in their daily living.
- Maintain accurate documentation of care plans, progress, home visits, and referrals.
- Monitor and evaluate the effectiveness of care plans, making adjustments as necessary.
- Act as a liaison between clients, families, healthcare providers, and community resources to ensure seamless service delivery.
- Participate in interdisciplinary team meetings and training programs to enhance service quality.
- Stay up to date on Advantage program policies and regulations.
Qualifications:
- Required: Bachelor’s degree and one year of paid experience working with seniors OR active RN or LPN license.
- Skills: Strong time management, communication, and documentation skills.
- Technology: Proficient in Microsoft Office
- Additional Requirements:
- Valid Oklahoma Driver’s License & Reliable Transportation for in-area travel.
- Proof of Auto Insurance.
- TB Test Verification or willingness to obtain annually.
