What are the responsibilities and job description for the Case Assistant - Community Transition Liaison Program position at Elder Services of Berkshire County?
The CTLP Case Assistant supports the Community Transition Liaison to operate at the top of their role and responsibility acting as an onsite point of contact in nursing facilities (NF) for residents, families, health care proxy (HCP), and NF staff related to transitions from NF to the community.
Education/Experience Requirements: High School diploma with 1 year of related work experience and/or interest in working with nursing facility residents including older adults or persons with disabilities who are transitioning from an institutional to a community setting.
Competencies:
- Excellent interpersonal skills; strong oral and written communication skills.
- Commitment to working with the elderly and disabled adults; strong sensitivity to aging and disability issues.
- Crisis intervention/problem solving skills.
- Ability to apply the practices, principles, and techniques used in professional social work.
- Leadership ability; independent thinker; self-starter.
- Proficiency in Microsoft office and experience using databases.
