We are currently hiring a Care Manager.
As a Care Manager , you will assess and monitor the needs of the consumer and implement a service plan which is following the Executive Office of Elder Affairs and Elder Services of Cape Cod and the Islands Home Care
Program regulations and contract obligations. This is a full-time position, Monday- Friday, 9-5, 34 hours / week plus unpaid one hour lunch each day. The pay rate for this position is $28.43 per hour .
