Eleos Family Services is a provider of Community-Based Support, Individual Supports Program, Behavioral Support Services, and Intensive In-Community Services in The Morris and Sussex counties of NJ. We are also looking for staff who live near the Tri-counties( Warren, Hunterdon, Somerset). We are dedicated to quality care for Adults with IDD and youth, adolescents, and their families through individualized, strength-based clinical interventions.
Eleos Family Services is looking for highly motivated and qualified partially licensed therapists to provide weekly in-person therapeutic counseling to youth 5-21 years of age, and families, in their home/community.
As a potential candidate, you will be expected to deliver customized intensive counseling to youth/adolescents to ensure they receive the necessary care and attention to overcome their challenges. With your background in the behavioral and mental health field, you will successfully execute the prescribed intervention strategies and document outcomes to facilitate the continuous improvement of services offered. The goal is to stabilize the youth in the home and strengthen/preserve the family constellation. There is no minimum caseload required.

Responsibilities:
- Conduct patient assessments and develop/implement treatment plans
- Follow protocols based on Medicaid guidelines and systems involved, if any, such as CSOC- Children System of Care.
- Must meet with the youth and their family in their home and/or community on a weekly basis.
- Provide In-Home/Community counseling (IIC) to children and adolescents who have emotional and/or behavioral needs.
- Provide behavioral interventions and behavioral management.
- Collaborate weekly with case managers on the status of family/youth's progress.
- Adhere to paperwork submission timeframes and deadlines.
Qualifications/Experience:
- Valid NJ clinical license: LAC, LSW LAMFT.
- Clinical Supervision hours for qualified candidates.
- State license in good standing to practice in New Jersey.
- 1 year of experience working with children/adolescents.
- Must have a valid driver’s license and a reliable registered and insured vehicle.
- Prior Intensive In-Community (IIC) experience is a plus.
