Community Engagement Navigator
Location: This position is primarily a Virtual/Remote role.
Work Shift Hours: Monday through Friday, 10:00 am to 7:00 pm (Central Standard Time).
The Community Engagement Navigator is responsible for engaging with members to remove barriers to Social Determinants of Health (SDOH) and acting as the communication liaison between the company and referral sources.
How you will make an impact:
Engages with members who have been identified as possessing significant SDOH barriers.
Works with the member to establish goals and next steps for addressing SDOH.
Supports member in selecting provider or programs most appropriate for supporting their needs.
Interfaces with care team to ensure that the individualized plan is aligned with existing or needed case management efforts.
Acts as a liaison between referral sources and the company.
Minimum Requirements:
BA/BS degree in Public Health, Social Work, Rehabilitation Counseling or related field And a minimum 2 of years’ experience with marketing, job development, job placement or social work; or any combination of education and experience which would provide an equivalent background.
Preferred Skills, Capabilities and Experiences:
Knowledge of long-term support services preferred.
Experience with geriatric population, serious mental illness, substance abuse disorder and intellectual disability.
