Job Summary
We are seeking a dedicated and compassionate Hospice Aide to join our hospice team. The ideal candidate will play a crucial role in patient care, ensuring that individuals receive the appropriate services and support throughout their healthcare journey. This position involves working closely with patients, families, and our RN team to implement effective care plans tailored to each patient's unique needs.
Duties
- Implement individualized care plans in collaboration with patients, families, RN's and healthcare providers.
- Perform tasks to care for the patients personal needs and facilitate appropriate care relationships .
- Coordinate care across various settings including clinics, primary care, and managed care environments.
- Maintain accurate medical records and documentation to ensure continuity of care.
- Advocate for patients and support them in services that are available.
- Regularly review patient progress as needed to achieve optimal outcomes with help of their RN case manager.
Requirements
- Current HHA license in the state of practice.
- Experience in patient care.
- Knowledge of medical records management and clinic operations.
- Familiarity with managed care principles and practices.
- Excellent communication skills for interacting with patients, families, and healthcare professionals.
- Ability to work collaboratively within a multidisciplinary team environment.
