Summary
The Home Health Referral Intake Coordinator is responsible for managing and processing incoming patient referrals for home health services. This role involves assessing referral documents, ensuring compliance with regulations, coordinating with clinical and administrative teams, and facilitating the smooth transition of patients into home health care services.
Responsibilities
- Review and process patient referrals from physicians, hospitals, and other healthcare providers to determine eligibility for home health services.
- Collect and verify patient information, medical history, and insurance details to ensure complete and accurate documentation.
- Coordinate with clinical teams, such as nurses, physical therapists, and physicians, to ensure timely patient intake.
- Communicate with patients and their families to explain the referral process, answer questions, and provide necessary documentation.
- Ensure compliance with federal, state, and company regulations and standards for home health care.
- Maintain accurate and up-to-date records in the patient management system.
- Work closely with insurance companies to verify coverage, obtain authorization, and resolve any issues related to reimbursement.
- Collaborate with the scheduling department to ensure timely visits from healthcare providers.
- Provide ongoing support to the patient during the intake process, ensuring a smooth and efficient transition into home care.
Qualifications
- High school diploma or equivalent; associate or bachelor’s degree in healthcare or related field preferred.
- Previous experience in healthcare intake, home health care, or medical office administration is highly preferred.
- Strong understanding of medical terminology and health insurance processes.
- Excellent communication and interpersonal skills to interact with patients, families, and healthcare providers.
- Attention to detail and ability to maintain accurate records.
- Strong organizational skills and the ability to handle multiple tasks simultaneously.
- Knowledge of HIPAA regulations and patient confidentiality standards.
- Proficiency in using electronic medical record (EMR) systems and Microsoft Office Suite.
Preferred Skills:
- Experience in home health care intake or referral processing.
- Familiarity with Medicare and Medicaid policies and procedures.
- Bilingual skills (e.g., Spanish) are a plus.
