Job Summary:
Provides direct care to residents while promoting residents’ highest level of functioning as directed by management. Assists in maintaining the resident's environment in a safe and neat manner. Promotes quality services within facility standards, State and Federal Regulations.
Essential Functions:
1. Receives and gives report to management on resident status at the beginning and end of each shift and as needed. Notifies the management of any changes in resident physical and/or mental status.
2. Provide personal care to residents per their individualized plan of care.
3. Documents all pertinent information regarding care as assigned (24-hour log).
4. Prepares residents for meals and assists to and from the dining room, assists with eating if necessary.
5. Assists residents with toileting and incontinence care.
6. Makes room checks and verifies resident location as directed by supervisor.
7. Keeps residents’ apartments neat and clean.
8. Performs weights monthly.
9. Completes prevention and restorative care including, but not limited to, ROM, ambulation, transferring, bathing, grooming and all other ADL care.
10. Maintains a working knowledge of facility infection control procedures.
11. Assists in turning, positioning and repositioning of residents.
12. Participates in interdisciplinary team processes including documentation of care delivered and resident conditions as directed by the supervisor as assigned. Attends care plan meetings as assigned and acts as resident advocate.
13. Communicates effectively with the residents, families, visitors, members of the health care team and other disciplines.
14. Actively participates in the facilities Quality Assurance Program.
15. Maintains education development and compliance with State and Federal Regulations and current knowledge of resident care practices.
16. Has a basic knowledge of the organization in order to answer basic questions.
17. Possesses working knowledge of facility emergency procedures including fire and disaster drills,follows facility policies.
18. Maintains resident confidentiality and adheres to HIPAA policies.
19. Performs other duties as assigned.
Education and experience
High school diploma or general education degree (GED). Six months’ experience in long-term care is preferred But not required.
