Job Description
Elevation Colorado seeks a Registered Nurse Case Manager to provide high-quality patient care and support services.
Key Responsibilities
- Promote and practice Elevation Hospice of Colorado's philosophy and mission of providing compassionate, comprehensive, and qualified care to patients with life-limiting illnesses.
- Assure the highest quality of care is provided and documented in accordance with the Plan of Care (POC).
- Perform initial and ongoing assessments of patients.
- Develop and implement POCs as prescribed by physicians and in conjunction with other IDG members.
- Provide education to patients, families, or caregivers on needs identified on the POC.
- Work closely with IDG members to provide all-inclusive physical, psychosocial, educational, emotional, and spiritual services to achieve the highest quality of care.
- Maintain primary control and professional management of each patient and act as liaison between physician, patient, family, and hospice team.
Requirements
- Registered Nurse with a current license to practice professional nursing in the state of employment.
- One year of experience in oncology, long-term care, hospice, or home health setting.
- Current Cardiopulmonary resuscitation (CPR) certification.
- Reliable transportation and valid driver's license.
