PACT Staff Responsibilities
- Provide comprehensive case management services as part of a multidisciplinary team.
- Mitigate housing issues as they arise, serving as the point person for consumers.
- Develop personalized treatment plans with consumers, ensuring their unique needs are addressed.
- Serve as primary case managers, adhering to Eliot's policies and PACT standards.
- Participate in completing assessments in a timely manner, assigned by the Team Leader or Lead Clinician.
- Evaluate client functions, strengths, and needs to provide effective support.
- Foster therapeutic connections with clients, offering clinical interventions to promote recovery.
- Offer problem-solving supports and interventions to address client challenges.
Schedule and On-Call Duties
Monday through Friday, 8am to 4pm, with one 12pm-8pm shift per week and one weekend per month. Additionally, rotating on-call duties and holiday schedules are expected.
