What are the responsibilities and job description for the PACT Housing Coordinator position at Eliot's?
Eliot's Behavioral Health Division provides an array of services to support individuals with severe mental illnesses and/or substance abuse disorders. Our PACT staff coordinates and delivers comprehensive recovery and rehabilitative services to consumers.
PACT Staff Roles
- Deliver case management services within a multidisciplinary team framework.
- Address housing concerns as they emerge, acting as the key liaison for consumers.
- Collaborate with consumers to develop tailored treatment plans, considering their distinct requirements.
- Act as primary case managers, upholding Eliot's policies and PACT standards.
- Contribute to assessment completion in a timely fashion, as designated by the Team Leader or Lead Clinician.
- Assess client capabilities, strengths, and needs to supply targeted support.
- Establish therapeutic relationships with clients, offering clinical interventions to facilitate recovery.
- Provide problem-solving assistance and interventions to assist clients in overcoming obstacles.
PACT Program Details
The program operates Monday through Friday, from 8am to 4pm, with one 12pm-8pm shift per week and one weekend per month. On-call responsibilities and holiday schedules will be rotated among team members.
