Position Summary:
We are seeking a dedicated and compassionate Registered Nurse to join our team. This role is focused on traveling to clients' homes to fill their medication dispensers, ensuring they receive the right medications on schedule. This position offers flexibility in scheduling, making it perfect for those seeking work-life balance. Each client interaction is typically one hour.
Key Responsibilities:
- Travel to clients’ homes to fill medication dispensers on a weekly, bi-weekly, or monthly basis, depending on individual client needs.
- Empower patients and families to reach healthcare goals by educating them on disease management.
- Ensure patients’ status is assessed on an ongoing basis, gathering and documenting pertinent information.
- Notify physicians of significant changes in patient status.
- Complete documentation and charting for all patient visits within 24 hours.
- Collaborate effectively with doctors and other healthcare professionals to ensure continuity of care.
- Provide compassionate care and build strong rapport with patients and their families.
Qualifications:
- Current Indiana Registered Nurse (RN) license.
- Prior experience in home health preferred.
- MatrixCare EHR experience preferred.
- Prior Authorization through Indiana Medicaid experience preferred.
- Valid Driver’s License and Personal Auto Insurance required.
- Clean driving record required.
- Computer/tablet proficiency required.
- Excellent communication and interpersonal skills.
- Ability to work independently while managing multiple patient cases effectively.
- Strong time management skills to maintain an organized, efficient schedule.
- Empathy and patience when interacting with patients and their families.
