FLEXIBLE SCHEDULING WITH NO WEEKEND REQUIREMENT
We are currently looking for a Home Health Case Manager RN that is interested in joining our team for Vigo county and surrounding areas.
Job Requirements and Responsibilities:
- Conducts in-person patient interviews and physical assessments
- Assures quality of care by measuring health outcomes against patient care goals
- Initiates and implements the plan of care and ongoing recertifications
- Communicates patient conditions and collaborates with appropriate providers to deliver care when patient needs evolve
- Initiates the plan of care and makes necessary revisions
- Provides education to patients and families on proper home health care procedures
- Responsible for the completion of all home health documentation, including admissions, recerts, discharges, and all OASIS documentation
- Conducts supervisory visits when necessary to monitor home health aide performance
- Work to decrease readmissions by promoting preventative care and ensuring continuity of care
- Serve as case manager for Medicaid patients.
Qualifications/Skills:
- Registered Nurse (RN) License, required
- One (1) year of professional experience practicing as a Registered Nurse (RN) in home health or similar setting; previous case management experience preferred
- Medicaid Home Health, preferred
