SUMMARY: The Children's Case Manager will provide case management/therapeutic services to youth with a severe emotional disturbance (SED). In coordination with therapeutic staff, the Case Manager will assist youth and families by providing information, education, and training to build knowledge, develop skills, and change attitudes with a goal of increasing the consumer’s independence, productivity, self-determination, integration, and inclusion in the community. The majority of the Case Manager’s time will be spent providing services to youth in the family home, educational setting, program site or other community settings, with minimal time spent at the Center. This position reports directly to the CBS Coordinator or designee.
MAJOR JOB RESPONSIBILITIES:
- Participates in treatment planning based on best practice theories and applies interventions
- Development of personal plan/goal sheet in conjunction with consumer's needs
- Obtains location and the process of acquiring resources; coordinates with area agencies
- Direct involvement in planning and advocacy in areas of: Education/Vocation, Residential, Financial, Health , Leisure, and Social Supports
- Education to family in symptomatology, awareness of relapse indicators and handling of crisis
- Assists in collection of program data and provides reports as necessary to immediate supervisor
- Works in conjunction with other mental health center staff
- Must provide transportation to and from various program activities
- (must maintain a good driving record)
- Meets direct service expectations of 47% of time worked
- Works evening hours each week unless direct service and client needs are met
- Meets deadlines for daily paperwork and reports
- Provides quality documentation
- Provides supportive intervention when needs arise outside of scheduled hours
- Assists other case managers and consumers as needed to help meet agency’s needs
- Performs other duties as deemed appropriate by immediate supervisor or designee
- Complies with ELC policies and procedures
REQUIRED QUALIFICATIONS:
- Bachelor’s degree or equivalent experience necessary to meet State of Kansas licensing regulations
- Possesses the minimum qualifications as documented in the Kansas Case Management Program Standards
- Familiarity with mental health theory and practice
- Clean driving record
- Incumbent must pass a security check as required by Medicaid Regulations
