Responsibilities of a Social Worker:
- Conduct psychosocial assessments of residents to identify their needs and develop appropriate care plans.
- Provide counseling and emotional support to residents and their families.
- Coordinate and facilitate support groups and educational programs for residents and families.
- Collaborate with healthcare professionals to ensure holistic care for residents.
- Advocate for residents' rights and ensure their needs are being met.
- Assist residents in transitioning to different levels of care or discharge planning.
- Maintain accurate and up-to-date documentation of resident interactions and care plans.
- Participate in interdisciplinary team meetings to discuss resident care and progress.
Qualifications of a Social Worker:
- Bachelor's or Master's degree in Social Work (BSW/MSW) from an accredited program.
- Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in Pennsylvania.
- Experience working in a healthcare or nursing home setting is preferred.
