What are the responsibilities and job description for the Emergency Housing Specialist position at Elk River Contracting Company LLC?
The Emergency Housing Specialist is a full-time position that includes case management and housing navigation with clients. This position works closely with the Executive Director, the Lead Service Coordinator, Emergency Shelter Coordinator, and the other Service Coordinators
RESPONSIBILITIES
1. Provide comprehensive service coordination services
- Complete Intake/Exit form
- Determine eligibility for program
- Complete homeless verification
- Complete Psycho-Social Assessment
- Develop goals (service plan) with program participants
- Develop and maintain working relationships with community agencies
- Provide information and makes referrals to community resources
- Assist with coordination of services
- Provide crisis intervention
- Complete vulnerability assessments fr Coordinating Entry process
Assist residents in locating and obtaining suitable housing
- Monitor and evaluate progress
- Update service plans
- Attend and participate in weekly case staffings (Staff Meetings)
2. Maintain resident records
- Maintain accurate client records following established guidelines and operation manuals
- Document all client contacts on Client Contact Form in a timely manner
- Maintain files
- Enter data in the Data Base (HMIS)
3.Assist in the day-to-day operation of the program
- Provide support for clients, assisting with guidelines and life skills
- Answer phone and take messages following Confidentiality Policy
- Provide customer service to clients
- Complete check-in processes
- Schedule and transport clients to health care/housing appointments
- Assist with other duties at the request of the Lead Service Coordinator, the Emergency Shelter Coordinator, or the Executive Director
4. Complete required agency documentation
- Complete and submit Time Log including all client contact
- Complete and submit Time Sheet
Requirements
- Ability to work effectively with the homeless population
- Ability to work independently and as a team
- Strong verbal and listening communication skills
- Ability to follow and enforce program guidelines
Education:
-preferred Bachelors degree in Human Services/similar fields or a combination of education/experience
