To be a successful Permanency Support Worker you must become able to do the following always:
- Ensure a trauma informed focus on creating a safe and supportive environment for staff and those served through knowledge, sensitivity, respect, and a focus on resilience and recovery.
- Provide direct supervision and support of supervisees utilizing the agency model for supervision, Supervision for Success.
- Coordinate and supervise client visits with families. Assist with completion of assessment packets and monthly contact. Provide parenting and life skills training and development. Implement tasks identified on case plans to help families. Attend case plan meetings as needed. Assist families with identifying, locating, and accessing basic resources and community support systems.
- Document 100% of direct services provided to the team staff assistant within one week of the occurrence. Submit required monthly reports to supervisor. Complete visitation record requests.
- Transport families and clients. Travel in state to assure client contact as needed.
- Collaborate with Permanency Specialist, Permanency Services Team, internal, and external stakeholders, demonstrating problem-solving/facilitation skills. Participate in individual and Permanency Services Unit/Dyad supervision once a month or as needed with the supervisor.
- Follow Company Policy and Procedure and operate according to the Company’s Program Practice Guidelines.

To qualify for this job, you need to have:
- High School Diploma or GED required. Bachelor’s degree in social worker or related field preferred.
- Work Experience - Prior experience working in Child Welfare, Human Services, or related field.
- 21 years of age. Must pass a background check, fingerprinting, and physical and drug screening.
- Maintain company-required training.
- Physical working requirements - Ability to travel, driving company or personal vehicle including at night. Ability to do occasional lifting of up to 50 lbs. Able to walk stairs. Ability to work flexible schedule, including some evenings and weekends. Employees are subject to outside environmental conditions occasionally, inside conditions frequently, with protection from weather conditions but not necessarily from temperature change.
