Director of Social Services Responsibilities:
- Develop and implement social services programs tailored to meet the needs of residents.
- Supervise and provide leadership to a team of social workers and support staff.
- Collaborate closely with interdisciplinary teams to develop and implement individualized care plans.
- Ensure compliance with state and federal regulations governing social services in healthcare settings.
- Conduct assessments, provide counseling, and support residents and their families.
- Manage discharge planning processes to ensure smooth transitions and continuity of care.
- Advocate for resident rights and promote a culture of ethical care practices.
- Participate in quality improvement initiatives and program development efforts.
Director of Social Services Qualifications:
- Must be a licensed Social Worker in PA
- Responsible, ready, and eager to work
