Job Overview
We are looking for a compassionate and dedicated Hospice Social Worker to join our team. As a Hospice Social Worker, you will provide emotional, psychological, and practical support to patients and their families during end-of-life care. Your role will involve helping clients cope with terminal illnesses, offering counseling, facilitating advanced care planning, and connecting families with appropriate community resources. This position is integral in ensuring patients and their loved ones receive the highest level of comfort and dignity in their final days.
Job Schedule
- The schedule will depend on the number of patients you are assigned. Typically, one patient is seen once a month for approximately one hour for an assessment.
- You will have the flexibility to schedule visits at times that are convenient for both the employee and the patient/family.
- This position offers a great deal of flexibility and the ability to manage your own time while ensuring patients receive the support they need.
Duties
- Conduct psychosocial assessments of patients and families to address emotional, social, and practical needs.
- Provide counseling and emotional support to patients and their families, and refer patient for bereavement counseling.
- Help patients and families navigate end-of-life planning, including advanced directives and funeral planning.
- Act as an advocate for patients' rights, ensuring their preferences are respected in their care.
- Collaborate with the hospice care team, including nurses, physicians, and chaplains, to provide holistic patient care.
- Identify and refer patients and families to community resources, such as financial assistance programs, support groups, or additional services.
- Assist in the development and implementation of personalized care plans for patients.
- Monitor the progress of patients’ psychosocial needs and adjust care plans as necessary.
- Provide support in crisis situations, ensuring emotional stability for both patients and families.
- Maintain detailed and accurate documentation in compliance with hospice policies and regulatory requirements.
Experience
- A Master’s degree in Social Work (MSW) from an accredited program.
- Valid state licensure in social work (LCSW, LMSW, or equivalent).
- Strong understanding of hospice care, grief, and bereavement processes.
- Excellent communication skills and the ability to work closely with patients, families, and a multidisciplinary care team.
- Compassionate, empathetic, and dedicated to supporting patients and families during end-of-life care.
- Ability to handle sensitive and emotionally challenging situations with professionalism and care.
