Empath Health is currently seeking a 340B Patient Care Coordinator to join our team. SUMMARY: The 340B patient care coordinator provides comprehensive care to clients and patients by assessing, planning, and advocating for their health care needs. The patient care coordinator will act as the liaison between EPIC's 340B program and the medical staff at their outposted healthcare facility. They monitor and coordinate patients' treatment plans, guide them to understand the status of their health, and check in on their progress. Compassionate, knowledgeable, and collaborative, the patient care coordinator also collaborates with other members of the care team to look after patients within the healthcare facility. |
POSITION QUALIFICATIONS/REQUIREMENTS: |
Education and/or Experience Bachelor's degree (B.A.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience. 2 - 3 years of experience in a health care setting preferred Minimum of one years' experience with 340B software, preferred Strong knowledge of Microsoft Suite products, specifically Excel ORGANIZATIONAL SUPPORT: * Demonstrate professional accountability. * Advocate for positive change * Display customer service excellence * Utilize all communication technologies and practices effectively. * Participate in improving performance and quality. * Manage resources effectively. |
JOB DUTIES/RESPONSIBILITIES: * Orients and educates patients on care coordination services and how it relates to EPIC and the 340B program; provides educational information in conjunction with direct care related to treatments and medications. * Provide oversight of the entire case and linkage to appropriate EPIC and other community services. * Assist HIV patients in seeking insurance assistance or other coverage for health care costs. * Provide continued support, education, and training to newly diagnosed patients or patients returning to care, to ensure access to and proper navigation of the HIV and STI care system. * Maintains knowledge of the guidance and policy changes that affect the 340B program, including, but not limited to, HRSA/OPA rules and Medicaid changes.
