Empath Health is currently seeking an Admissions-Hospital Team Coordinator to join our team.
The Admissions and Hospital Team Coordinator supports and works with stakeholders across all hospice settings, home health, and palliative care within Suncoast Hospice, Hillsborough Hospice, and Empath Hospice, as well as Tidewell Hospice as needed to coordinate and ensure timely patient access to care. Administrative responsibilities include, but are not limited to, communication, scheduling, auditing for compliance for revenue billing/regulatory requirements, and creating/distributing reports on an ongoing basis.

Qualifications/Requirements
- Education and/or Experience: Associate degree in business, health administration or other related field; or two years related experience and/or training; or equivalent combination of education and experience.
- Excellent verbal and written communication skills.
- Solid customer service skills. Must be able to interact with in business meetings with referral sources and business development.
- Excellent documentation and organizational skills, including time management, multi-tasking and prioritizing.
- Exhibit self-direction, independent judgement, discretion and ability to work and collaborate with all team members including volunteers.
- Strong computer knowledge and ability to type working in MS WORD, EXCEL, PowerPoint and Outlook and the ability to learn additional software as needed including the Electronic Medical Record.
- Sensitivity to the needs of the staff, patient, caregivers and family members and demonstrates flexibility with system/processes to meet those needs.
- Medical terminology background required, knowledge of CMS hospice regulations and operations preferred
