Service Coordination: For the NYS TBI/NHTD Waiver Program
The Service Coordinator is responsible for assuring that the Service Plan is implemented appropriately and supporting the participant to become an effective self-advocate and problem solver. Together they work to develop and implement the Service Plan, which reflects the participant’s goals. The Service Coordinator assists the participant in the development, implementation and monitoring of all services in the Service Plan. Additionally, the Service Coordinator must initiate and oversee the assessment and reassessment of the participant’s level of care and on-going review of the Service Plan.
A Service Coordinator must be knowledgeable about all waiver services, Medicaid State Plan Services, and non-Medicaid services. Informal supports are often a crucial factor if the participant is to live a satisfying life and remain in the community. The Service Coordinator’s ability to make use of these informal supports is essential, and offers the Service Coordinator and other providers the greatest opportunity for creativity.
A BA is required and 1 to 2 years experience.
Community Integration Counselor for the NYS TBI/NHTD Waiver Program
CIC is an individualized service designed to assist the TBI or NHTD waiver participant to more effectively manage the emotional difficulties associated with adjusting to and living in the community. It is a counseling service provided to a participant coping with altered abilities and skills, the need to revise long term expectations, and changed roles in relation to significant others. This service is generally provided in the provider’s office or the participant’s home. It is available to participants and/or anyone involved in an ongoing significant relationship with the participant when the issues to be discussed relate directly to the participant.
A Masters Degree is Required and 1 to 2 years experience.
