Empire Spanish Meadows is looking for a registered nurse to become their MDS coordinator! The ideal person will have the ability and willingness to set goals, monitor progress and obtain established goals within the established time frames. Job Requirements:
- Schedules residents for appropriate admission, annual, quarterly, Medicare assessment or change-of-condition assessments, overseeing communication to nursing and other departments.
- Monitors MDS and care plan documentation on all residents, assuring that complete, orderly chronological and current documentation is in file.
- Performs care plan updates as needed.
- Provide DON, Administrator and pertinent staff with the Quality Indicators Report.
- Perform transmissions of the MDS on a timely basis.
- Remains current with changes in the industry through attendance at outside workshops and conferences.
- Participate in Triple Check Meetings.
- Assist with ABN’s, Physician Certification and Recertification’s.
- Review of ADR documentations.
- Perform other job-related duties as assigned.
- Coordinate and attends regular Medicaid meetings to discuss current residents, pending residents and recipients.
- Coordinate and attends all IDT meetings in order to gather information, communicate changes, maintain and update the clinical records including the Care plan.
- Assures compliance with Federal and State regulations regarding completion / coordination of the comprehensive assessment (MDS).
Education/ Experience/ Licensure Requirements:
- Licensed LVN or RN in good standing
- Ability to work independently as well as a meaningful member of the team.
- Ability and willingness to set goals, monitor progress and obtain established goals within the established time frames.
- Typing, telephone and basic computer skills.
- Ability to perform work tasks within the physical demand requirements as outlined below.
- Genuine caring for and interest in elderly and handicapped people.
- Maintains proficiency in software.
Skills:
- Excellent organizational skills in order to coordinate many projects simultaneously.
- Restorative care philosophy, excellent professional skill (both assessment and hands-on and good communication and organization skills).
Benefits:
- Competitive pay - up to $82,000/year- depending on experience

Physical Demands:
The employee must be able to perform the essential functions of the position satisfactory and if requested, reasonable accommodations will be made to enable employee with disabilities to perform the essential function of their job absent undue hardship. This position does require employee to be able to frequently travel, walk, lift to 25lbs, sit and stand.
Reliability, trustworthiness, and consistency with regard to attendance is extremely important to this job. The ability to regularly and timely attend work, cooperative and politely work and deal with others, and to effectively multi-task and work in a stressful environment are also essential functions to this job.
