Unique opportunity to join a dynamic Ambulatory Care team in value-based health care system. We are an integrated health care delivery system, which includes Primary Care and medical specialties, onsite pharmacy, radiology, laboratory, dental, physical therapy, eye care, and mental health services.
Workweek includes 4.5 days of direct patient care and 0.5 day administrative time, with exclusively ambulatory medicine practice, no nights or weekends and no on-call coverage. Please note you would be expected to work at both our Midtown and Harlem Health Centers.
Duties and Responsibilities:
- Provide comprehensive care within the scope of endocrinology in collaboration with the care team
- Ensure complete and timely documentation of all pertinent information in the electronic medical record.
- Collaborate with other health care professionals as needed to coordinate comprehensive treatment plan
- Communicate with the patient on results and appropriate follow up steps
- Provide patient education in collaboration with nursing staff
Required Qualifications:
- BC in Endocrinology
- Active, unrestricted New York State Physician License
- DEA Certificate
- BCLS Certification
- Infection Control Certificate
- Completed training in the identification and reporting of child abuse as per NYS requirements
- Completed NYS Department of Health mandated prescriber education in pain management, palliative care, addiction within the past 3 years
- Malpractice Insurance in the amounts of a minimum of 1M/3M, issued by a carrier authorized to issue policies in NYS
