Human Services Clinician

Employment Enterprises, Inc. is working with a government entity that supplies a variety of funds for services and programs in our local area. The Human Service Clinician would work in the Adult Services and Adult Protect Services units to facilitate independent living in the least restrictive setting possible for older adults and adults with disabilities and prevent and reduce abuse, neglect, and exploitation of vulnerable adults. This position is onsite in Arlington, Virginia and pays $24 per hour.

Human Services Clinician Responsibilities

• Receives reports and determines validity of allegations of abuse, neglect and exploitation of adults ages 60 and above, and adults ages 18-59 with a permanent disability.
• Conducts investigations of abuse, neglect, and exploitation by conducting home visits, interviewing involved parties and making dispositions.
• Completes face to face visits with clients within specific timeframes, coordinates home and community-based services and collaborates with others to assess risk and improve the health and safety of clients.
• Performs comprehensive assessments using the Virginia Uniform Assessment Instrument (UAI) to assess physical and mental health, cognitive functioning, financial situation, and formal/informal support systems.
• In collaboration with the client, develops a person-centered, trauma informed, solution focused care plans to prevent/reduce abuse, neglect, or exploitation.
• Monitors the wellbeing of affected individuals and arrange other care services as needed.
• Works with multidisciplinary teams to remedy unmet needs and reduce behavior patterns of risk, crisis stabilization, and solicit family/caregiver cooperation in care planning.
• Maintain a caseload of clients at risk of abuse, neglect and exploitation.
• Evaluate and assess clients at risk of abuse neglect and exploitation and implement care plans to improve the safety of these clients.
• Evaluate the progress of care plan objectives outlined, modify the plan as needed, and close case when objectives are achieved,
• May complete and participate in court processes, including but not limited to guardianship/conservatorship hearings.
• Offer alternatives to living environment if necessary and when appropriate, that may include placement of the adult client in an alternative living environment or long-term care residence.
• Ensure effective and timely documentation of all case notes and records. Maintains electronic case records in compliance with applicable clinical and regulatory requirements.
• Participate in after hours on call responsibilities as scheduled.
  
  

Human Services Clinician Qualifications

• Providing services to persons with disabilities, older adults (60 ), and their families.
• Experience in Adult Services, individuals with SMI and/or experiencing homelessness, or other case management practice preferred.
• Collaborating with Courts and the legal system when providing services.
• A clinically proven team player who brings a passion for working with older adults with a strength-based and person-centered approach in human services delivery.
• Clinically-sound, unprejudiced and adapt at functioning within a multi-disciplinary environment that believes in applying strengths-based and person-centered approaches to the delivery of human services.
• Working with individuals from diverse cultural/ethnic backgrounds.
