What are the responsibilities and job description for the DIRECT SUPPORT PROFESSIONAL position at Employment Solutions Inc?
Specific Duties:
1. Provide direct support for waiver participants. These duties include but are not limited to: completion of
monthly service summary, participation in plan of care and team meetings, completion of regulatory required
reports, attending to the concerns of family and team members, and advocate for the general welfare of waiver
participants.
2. Plan, arrange and conduct activities which enhance self-sufficiency, self-improvement, independence building,
socialization, relaxation, and community involvement.
3. Engage waiver participants in activities throughout their scheduled time of attendance.
4. Provide assistance with ADLs such as toileting, feeding, continence care, dressing, etc.
5. Maintain a safe and respectful environment for participants.
6. Perform medication administration duties for day training participants.
7. Transport waiver participants to community activities.
8. Other duties as assigned.
Minimum Requirements:
1. Meets the personnel and training requirements.
2. Has the ability to communicate effectively with a participant and the participant’s support team; including reading,
writing, and implementing written and oral instructions, and performing required documentation.
3. Personal, reliable vehicle and valid driver’s license
4. Valid car insurance
5. Ability to physically perform activities and duties as assigned.
6. Must be 18 years of age with a high school diploma or GED OR be at least 21 years of age.
