Team Collaboration
Collaborate with Case Managers, Recreation Coordinator, and Shift Coordinator to provide direction for resident activities
Collaborate with Case Managers to interpret goals from Individual Service Plans for use with Daily Documentation
Attend Behavior Intervention Planning (BIP) meetings, MDT meetings, and any mandatory training, as directed
Assist in therapeutic interventions of residents
Welcome & mentor new staff, serve as a role model
Assist in the development and coordination of recreational activities
Serve on various committees for Performance Improvement & Individual Career Path
Safety & Supervision
Monitor the whereabouts of residents at all times according to the agency’s policy and procedures
Maintain knowledge of all resident information sheets and resident safety plans including 1 : 1
Provide Adult / Child CPR / First-Aid during emergency situations
Provide appropriate SCM interventions according to their certification, when a resident is harming himself, others or when imminent harm is present
Ensure proper resident hygiene of assigned residents
Data Collection & Analysis
Track and calculate Daily Documentation scores and updates the boards as residents progress
Review Behavior Management of residents in assigned cottage / s
Complete required daily documentation such as Behavior Tracking, Incident Reports, Daily Documents, and any other applicable forms
All other duties, as assigned by Residential Shift Coordinator, Director of Residential Services, Education Shift Coordinator or Residential Services Coordinator
Position Qualifications :
High School Education or equivalent (Associates Degree Preferred)
8-12 months prior experience working with target population preferred
Maintain Skill Builder 3 or 4 qualifications
Adult / Child CPR / First Aid and SCM Certified
Knowledgeable computer skills including accessing email, internet & Microsoft office
Interact in a pleasant and professional manner with other employees, physicians, providers, agencies, residents, and resident’s families to promote a team effort through cooperation and respect for fellow employees.
May be exposed to fumes / odors, body fluids / blood / tissue.
May be required to adhere to resident safety coverage policy.
Maintain annual TB tests, required paperwork and background screenings as required by licensing agency.
Complete required hours of Orientation Training and any other training required by regulations, which will include Safe Crisis Management training. Skill Builders will be required to pass the SCM written test as well as the physical test and attend twelve hours of annual ongoing refresher training along with passing the physical and written exams annually.
