Role Description:
This is a part time or full-time hybrid role for a Program Supervisor role located in Bloomington, MN. The Program Supervisor will be responsible for overseeing and coordinating various projects, developing and implementing plans, and ensuring project timelines are met.
- Position with occasional evening and weekend hours as needed to meet client needs.
- Some travel within Minnesota may be required to meet clients or attend meetings.
- Flexible working environment with remote work.
Team Collaboration and Leadership:
- Work closely with direct support professionals, caregivers, and other team members to ensure that services are delivered consistently and effectively.
- Coordinate with other departments and service providers to ensure comprehensive care for clients.
- Provide guidance, training, and support to staff involved in the delivery of services, ensuring adherence to best practices and regulatory requirements.
Service Monitoring and Quality Assurance:
- Regularly monitor the delivery of services to ensure that all activities are aligned with the established care plans and in compliance with 245D waiver regulations.
- Identify any gaps or areas for improvement in service delivery and work to address them proactively.
- Document and track client progress, ensuring that all information is updated and accurate within client records.
Resource Coordination:
- Connect clients with community resources, services, and benefits that can enhance their overall well-being and help them achieve their goals.
- Stay informed about local services, government programs, and other support systems to offer clients a wide range of resources.
- Facilitate communication and coordination between clients and service providers, ensuring that all necessary supports are in place.
Compliance and Reporting:
- Ensure that all services and documentation meet state and federal regulations, including 245D waiver compliance.
- Maintain accurate and up-to-date client records, including care plans, assessments, progress reports, and other necessary documentation.
- Assist with reporting requirements and audits, ensuring compliance with agency policies and procedures.
Qualifications:
- Bachelor’s degree in social work, human services, or a related field (preferred).
- Minimum of 2 years of experience in client support, or working with individuals with disabilities.
- Strong knowledge of 245D waiver services and Minnesota disability services regulations.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently, prioritize tasks, and manage multiple cases.
- Compassionate, client-centered approach with a commitment to improving the lives of individuals with disabilities.
- Experience with care planning, assessment, and documentation is a plus.
