Description

Program:

Enhanced Care Management (ECM) Core Services include outreach and engagement, comprehensive assessment and care management planning, enhanced coordination of care, health promotion, transitional care services, member and family supports, and coordination of and referral to community and social support services.

Community Support (CS) Services offered through L2H are housing transition navigation services, housing deposits, and housing tenancy and sustaining services. Other CS providers in the network may offer other allowable CS services such as recuperative care, respite services, personal care and homemaker services, nursing facility transition/diversion to assisted living facilities, medically-supportive food/meals, and asthma remediation. Encompass CS providers will support with all CS linkages.

As defined by the California Department of Health Care Services (DHCS), Populations of Focus include individuals experiencing homelessness, persons at risk for avoidable hospitalization or ED use, persons with Serious Mental Health and/or SUD needs, individuals transitioning from incarceration, adults living in the community and at risk for long-term care institutionalization, adult nursing facility residents transitioning to the community, children and youth enrolled in CCS or CCS WCM with additional needs beyond the CCS condition, children and youth involved in child welfare, and birth equity.

Responsibilities:

 

  • Oversees and manages all aspects of Encompass Links2Health case manager program in outpatient and residential treatment programs at both our North and South County locations.
  • Provides management and supervisory oversight of approximately 10 or more case managers.
  • Participates in staff recruitment, completes interviews on interview panels, and follow up hiring processes.
  • Manages staff onboarding, training, coaching, coordinating and processing staff time off requests, reviewing and approving time cards to ensure accuracy, and conducting any possible progressive disciplinary action for staff.
  • Meet and provide direct, one-on-one supervision to case managers (minimum of 2 times monthly) to review work performance and provide coaching and training support to staff. 
  • Hold and facilitate weekly L2H team meetings.  Relay any relevant program agenda items to the Links2Health Program Leadership team meeting monthly.
  • Arrange and ensure proper case manager coverage as needed.
  • Observes sessions at least one per quarter for each staff to identify strengths in therapeutic intervention services and areas for improvement.
  • Ensure NOABDs are in compliance.
  • Review staff billing and units of service productivity on a weekly basis to ensure monthly billing goals are met and provide feedback/coaching support to staff as necessary.
  • Ensure that staff remain active in their certifications and registrations and complete their renewal requirements on time.
  • Attend county annual site visit.  Write plan of correction when needed with support from the Senior Manager.
  • Ensures all charts and documentation is maintained timely and accurately.
  • Complete monthly reports and submit to the Senior Manager for review
  • Maintain active clinical licensure
  • Offer case management support through our integrated model of Enchanced Care Management (ECM) and Community Support (CS) program.

 

Qualifications:

A Master's degree listed below is greatly preferred: 

  • Master’s Degree in Social Work (MSW) with a focus on Clinical Social Work: Advanced training in social work with an emphasis on clinical practice, equipping candidates to manage case management programs and support clinical staff.
  • Master’s Degree in Public Health (MPH): Education in public health principles, policies, and management, ideal for overseeing clinical programs aimed at improving community health outcomes.
  • Master's Degree in Healthcare Administration (MHA): Advanced education focusing on the management and administration of healthcare organizations, preparing candidates for leadership roles in clinical settings.
  • Master's Degree in Nursing (MSN): Specialized education for nurses, providing the knowledge and skills necessary for higher-level management and clinical oversight roles.

Experience in the following areas required: 

  • Extensive Clinical Experience: Proven background in clinical settings with experience in managing and overseeing patient care plans, ensuring high-quality standards and compliance with healthcare regulations.
  • Leadership and Team Management: Strong leadership skills with a track record of effectively managing small teams of case managers, providing guidance, support, and professional development opportunities.
  • Program Development and Implementation: Expertise in designing, implementing, and evaluating clinical programs, with a focus on improving patient outcomes and streamlining case management processes.
  • Strong Communication and Interpersonal Skills: Excellent communication abilities, both written and verbal, to effectively interact with team members, healthcare professionals, patients, and their families.
  • Data-Driven Decision Making: Proficient in using data and analytics to monitor program performance, identify areas for improvement, and implement evidence-based strategies to enhance program effectiveness
