SUPPORT COORDINATOR II – OLDER ADULTS DIVISION
Encompass Community Supports, formerly known as Rappahannock Rapidan Community Services (RRCS) is hiring a Support Coordinator to work in our Community Connections for Older Adults Division.

At Encompass Community Supports, our full-time employees work 37.5 hours weekly.
Our Support Coordinator will assess clients over the age of 60 years old by identifying needs and providing access to appropriate services including aging related programs from our agency and other agencies including service linkage, case monitoring and client advocacy functions. Work is performed under regular supervision.
SOME OF THE RESPONSIBILTIES:
- Assesses client needs, presents options and services based on a needs assessment; develops a plan of care with client and/or family members when appropriate.
- Develops and implements service delivery, makes referrals and linkages to the proper agencies.
- Uses Electronic health records to input client’s information; maintains and updates files, records of actions taken, and care plans as appropriate.
- Makes home visits to evaluate client environment for safety, security and negative factors, and updates service delivery as needed.
- Provides one on one Medicare, Medicaid, and LTC Insurance counseling (VICAP) and enrolls clients in Medicare D, extra help and assists with other human services applications (SNAP, Medicaid and Medicare Savings Programs).
- Participates in staff meetings, community groups and other conferences as they relate to staff, client and program needs.
- Performs other duties as assigned.
KNOWLEDGE, SKILLS AND ABILITIES:
- General knowledge of social case work and interviewing and supportive counseling techniques.
- General knowledge of the theories, principles and techniques of individual, family and group therapy; ability to analyze facts and exercise sound judgment in arriving at conclusions. Ability to establish and maintain effective working relationships with clients, associates and the general public.
REQUIRED QUALIFICATIONS:
- Bachelor’s degree in Human Services or equivalent;
- At least 1 year working with older adults.
- Valid Virginia driver’s license.
- Employment with Encompass Community Supports is contingent upon the successful completion of our background check process (criminal, DMV, education, drug screening, reference check, DSS, OIG and SAM).
PREFERRED QUALIFICATIONS:
- At least 2 years’ experience in case management
