What are the responsibilities and job description for the Admissions Coordinator, SNF position at Encore Village of Schaumburg?
Encore Village of Schaumburg is currently hiring for a full time Admissions Coordinator in our skilled nursing facility. The Skilled Nursing Admissions Coordinator will serve Encore Village by communicating with referral sources, physicians, third party providers, agency peers, and clients to facilitate a thorough and complete admission process. Eligible for full time benefits effective 60 days plus 1st of month from date of hire.
Essential Duties and Responsibilities :
- Receive referrals, physician orders, insurance coverage, and all clinical data from referral sources.
- Communicates referral information to the Marketing Clinical Liaison and / or Admissions Director.
- Maintain accuracy in obtaining and recording patient, financial, and clinical information.
- Knowledge of health insurance, Medicare, Medicaid, and managed care reimbursement guidelines and regulations.
- Confirming insurance coverage, benefits, authorization and / or predetermination for care to be provided, as needed.
- Responsible for room assignments.
- Responsible for room readiness in accordance to protocol.
- Perform tours as needed for a back up to the Admissions Director.
- Communication with patients and their families to ensure a smooth admission process.
- Communication with all departments within the Skilled Nursing Facility to ensure a smooth admission process.
- Preparing and signing the appropriate Admission Agreements and accompanying forms in a timely manner to ensure compliance with Admission Agreement Policy.
- Support ongoing Quality Improvement activities to ensure quality patient care.
