What are the responsibilities and job description for the Life Services Coordinator Assisted Living position at Encore Village of Schaumburg?
Encore Village of Schaumburg is currently hiring for a full time Life Services Coordinator in Assisted Living. The Life Services Coordinator is responsible for assessing Assisted Living external applicants and residents from The Willows Assisted Living and Reflections Memory Support, to determine appropriate level of living, movement through the continuum, and services needed. Knowledge of available support services within both general assisted living and memory support, as well as outside of. Possesses strong ability to critically think and implement various interventions for residents/families. Works with residents and families in collaboration with the Assisted Living Team to resolve issues/concerns and provides basic case management services to residents.
Responsibilities include:
- Case Management (All PHI): providing intervention as needed throughout the community with an emphasis on Assisted Living residents. This is accomplished through:
- Maintaining current knowledge in area of geriatric care.
- Appropriate application of various assessment tools.
- Works closely with the Marketing Department to ensure positive experience for applicant and family throughout their approvals process into the community.
- Conducts assessment of applicants to determine needs, appropriate supportive services, and level of living.
- Creates Service Plan and plays role, along with Assisted Living staff in implementation of plan.
- Coordinates referrals to nursing staff and/or other supportive services as needed.
- Follow up with new resident in a timely manner after move-in to evaluate adjustment to environment or need for additional support.
- Identifies need for additional assessment of Assisted Living population.
- Review assessment findings with resident and/or family and creates a plan of care with input from the RN Management.
- Acts as an advocate for all Assisted Living individuals.
- Accessible to residents and families, counsels as needed, and refers to appropriate resources via telephone, email and meetings.
- Refers residents and families to appropriate resources as needed.
- Ability to act as a First Responder and assist in crisis events as needed.
- Interacts regularly with various physicians and clinicians.
- Identifies and coordinates psychological interventions.
- Provides, explains, and assists residents with Advanced Directives.
Transitions:
- Appropriate identification of need for transition to higher level of care, or Briarwood Health Care Center or Hospital.
- Knowledge of admission process to various areas including regulations, policies and procedures and needed documentation.
- Manages transition process in conjunction with residents, associates, families, clinicians.
- Establishes a strong, professional relationship that promotes open communication with various case managers throughout Encore Village and within the community.
- Provide follow up care after hospitalization or rehabilitation stay.
Regulatory Compliance:
- Works collaboratively with Privacy Officer to ensure HIPAA compliance at all times.
- Reviews all incident reports and proides follow-up documentation directly to Assisted Living Director.
- Identifies and addressed risk and safety concerns/issues.
- Ensure resident meets IDPH criteria to reside in Assisted Living.
- Knowledge of Independent Living, Assisted Living, and Healthcare IDPH regulations.
Education/Experience:
- Bachelor's Degree in Social Work or related field
- Previous social service experience, preferably in long term care/retirement community setting.
- Experience working with the elderly population.
