POSITION PURPOSE:
Provides specialized clinical oncology pharmacy services to patients in designated specialty patient care areas and coordinates these services with other healthcare practitioners. Performs duties of a staff pharmacist as required.

POSITION PURPOSE:
Provides specialized clinical oncology pharmacy services to patients in designated specialty patient care areas and coordinates these services with other healthcare practitioners. Performs duties of a staff pharmacist as required.
ESSENTIAL FUNCTIONS:
- Manages drug therapy treatment plans and protocols as a part of an interdisciplinary team.
- Reviews, interprets, monitors, and evaluates medication orders to identify, prevent, and resolve potential or actual drug related problems.
- Provides drug information and education to medical staff and patients.
- Documents interventions and their outcomes for use in drug therapy quality improvement efforts.
- Review, assess, and assist in maintaining compliance with regulatory standards.
- Assist pharmacy buyer in maintaining oncology drug inventory and help navigate drug shortages.
- Consults with other department directors, hospital administration and other hospital personnel as necessary or designated by the director.
SPECIALIZED KNOW-HOW AND REQUIREMENTS
- Bachelor???s degree in Pharmacy required. PharmD degree preferred.
- Current Illinois Registered Pharmacist License required.
- Completion of PGY-2 Oncology Residency or a minimum of 5 years non-dispensing oncology experience required
- Board certification in oncology pharmacy specialty (BCOP) required within 18 months of hire
- Demonstrated computer literacy with in-depth knowledge of Windows and pharmacy software applications.
- Excellent interpersonal, organizational, oral, and written communications skills required.
- Demonstrates the knowledge necessary to provide care appropriate to any age-related needs of the patients served on assigned unit. Demonstrates knowledge of the principles of growth and development appropriate to the patient population served. Able to assess and interpret data about the patient???s status to identify each patient???s needs and provide the appropriate care, including age-related care, needed by the neonate, pediatric, adolescent and/or geriatric patient group.
