Position Purpose:
The Community Health Worker (CHW) Team Lead supervises the completion of all tasks assigned to the CHWs and supports the manager and team. The Team Lead works with the manager to answer staff questions, provide training and guidance, and ensure timely and accurate completion of reporting. The CHW Team Lead oversees and supervises the daily activities of a team of Community Health Workers, engaged in providing education, care coordination, and access to resources to members to improve their health and well-being. Additionally, they serve as the liaison to health care system payers; assist with public and potential presentations; and work in both clinical and community settings, including member’s homes.
Competencies/Duties:
- Team Lead oversees all facets of the daily operations of the Community Health Workers, to include functional coordination and administration of related work performance.
- Supervises Community Health Workers, to include work allocation, training, and problem resolution; evaluates performance and makes recommendations for personnel actions; motivates team members to achieve peak productivity and performance.
- Assists in the creation of training modules and continuing education curriculum for Certified Community Health Workers.
- Trains new Community Health Workers and provides ongoing staff development instruction.
- Conveys all questions and concerns of Community Health Workers regarding clients and plan of care to Community Health Worker Team Lead.
- Team Lead reviews reports for clarity, content, spelling, grammar and follow through on member’s plan of care.
- Assist Community Health Workers in taking on more complex cases as needed. Includes assisting members in their homes, community, or clinic setting, documenting member encounters, maintaining member files and documenting activities.
- Educates and assists members in accessing health related and social services, provides support and advocacy and facilitates access to community resources.
- Works collaboratively and effectively with Community Health Workers:
- Establishes positive, supportive relationships with members and provides feedback to other members of the team. Builds and maintains positive working relationships with the members, providers, nurse case managers, agency representatives, supervisors, and office staff, from diverse cultural and socio-economic backgrounds.
- Works to reduce cultural and socio-economic barriers between members and institutions.
- Assists with public presentations on programs to interested audiences.
- Team Lead travels to client homes, community locations, various agencies, and other outreach destinations.
- Performs miscellaneous job-related duties as assigned.
Requirements
Position Requirements/Abilities:
- Locate and engage specific Members referred based on gap in care list for identified priorities;
- Assess the current state of the Member’s environment and lifestyle- identifying risk factors that are contributing to the Member’s gaps in care;
- Partner with the Member to problem solve and address barriers to desired health outcomes;
- Provide information and education that promotes the Member’s engagement in their health and utilization of available benefits and services;
- Promote key Customer resources for Member use including Care Coordination, enhanced benefits and services, and incentive programs when appropriate;
- Work collaboratively with Customer staff on an ongoing basis to coordinate efforts in successfully assisting Member in obtaining needed services, meeting telephonically, electronically and/or in person with Customer staff as appropriate;
- Provide support and resources to Member caregivers;
- Team Lead connect Member with home and community-based services;
- Interact with Member until all needs are met and/or Member has related to care coordination;
- Address Social Determinants of Health needs and ensure Member is connected to all providers as needed.
Education, Work Experience, Licensure:
- One of the following certifications/licensures preferred: Community Health Worker, Recovery Specialist, Peer Recovery Support Specialist, Licensed Clinical Social Worker
- Ability to obtain a valid Indiana driver’s license, proof of valid insurance, and access to reliable transportation.
- High School Diploma or equivalent GED
- One-three years of experience in health and human services, Public Health, Medicaid, Medicare, Managed Long Term Supportive Services, or behavioral health field working in a diverse community-based setting.
- Bilingual candidates preferred.
- Working knowledge of community-based populations, medical conditions and/or disabilities
Environment/Physical Demands:
- Ability to continuously stand or walk. Ability to bend, climb stairs with the potential for frequent lifting
- Ability to lift to 25 pounds, occasionally.
- The remote position requires the individual to work from home when not meeting with members in the community . Must have dedicated space within the home to operate fully.
- This position requires frequent driving which requires the ability to sit for long periods of time. Must provide proof of a valid Driver's License and valid insurance coverage.
- This position requires the employee to conduct home visits in members’ homes which requires the individuals to obtain and exchange information and assist the member with resolving simple to complex issues related to their accessing their health care benefits. The work requires frequent communication which requires ability to express self and receive information.
