Overview
Enhabit Home Health & Hospice is hiring a Central Intake Coordinator to join our team in Alabama. The Central Intake Coordinator is responsible for regional coordination of patient referrals and ensures that adequate transition to home health services occurs.
Hours: Monday - Friday, 8:00am - 5:00pm
The Central Intake Coordinator can work from one of these offices:
Jasper Office: 3699 Industrial Pkwy, Jasper, AL 35501
Hoover Office: 1 Chase Corporate Dr., Hoover, AL 35244
Responsibilities
- Receive, document, and appropriately communicate referrals from physicians, facilities, and staff.
- Initiate contact with patient to verify awareness of referral, to introduce Enhabit, and to verify information
- Ensure patient demographic information is accurate on the referral
- Verify Medicare eligibility and ensure accuracy of Medicare number in patient record
- Ensure the appropriate branch office receives referral information, including accurate and current dates of admission
Qualifications
- Must have a high school diploma or equivalent.
- Must have one year of previous experience in home health. Three years of home health experience is preferred.
- Demonstrated knowledge of Medicare guidelines governing home health is required.
- Must be organized, detail-oriented, and able to communicate effectively.
- Must have strong customer service skills.

Compensation Range: $16-$18 per hour
