Job Description: Clinical Team Leader
The Clinical Team Leader assists the Branch Director in ensuring overall coordination of home health services to clients is delivered according to acceptable standards of practice. The CTL is responsible for reviewing and approving patient information submitted by Licensed Professionals during Start of Care, Recertification, Resumption of Care, or Evaluation visits.
The CTL oversees patient care, assists with case conferences, reviews and approves orders, and provides education and training to branch clinicians on documentation requirements to meet regulatory standards. The CTL promotes effective relationships between healthcare providers, patients, and caregivers while maintaining regulatory compliance.
The CTL role may require participation in an on-call rotation and making patient visits. To excel in this position, you will have:
- A graduate degree in nursing from an accredited school and licensure in the state of employment and where practicing
- A minimum of two years' experience in nursing and one year in home health
- A good understanding of Federal, State, and local laws governing Home Health agencies
- Skilled competency with technology, including general office equipment and EMR platforms, preferably HCHB
- A valid state driver's license and automobile liability insurance
- The ability to drive safely in various weather conditions
