As a Clinical Team Leader with Enhabit Home Health & Hospice, you will be responsible for the overall coordination of home health services to all clients, delivered according to acceptable standards of practice. This includes assisting the Branch Director with day-to-day office and staff management related to patient care, providing ongoing education and training to all branch clinicians, and contributing to the overall company success of providing high-quality patient care.
Key Responsibilities
- Electronic review and approval of patient information submitted by licensed professionals during start of care, recertification, resumption of care, or evaluation visits
- Participation in case conferences, review and approval of orders, and patient care oversight
- Staff management, including ongoing education and training for clinicians
- Facilitating relationships between physicians, referral sources, patients, caregivers, and employees to contribute to the company's success in providing high-quality patient care
Requirements
- Graduate of an accredited school of professional nursing and licensed in the state of employment and where practicing
- A minimum of two years' nursing experience and one year's experience in home health
- Good understanding of Federal, State, and local laws/regulatory guidelines governing the operation of a Home Health agency
- Skilled competency with technology, general office equipment, and EMR platforms, preferably HCHB
