What You’ll Do:
- Provide case management for HUD programs, ensuring compliance with DMHAS CSS guidelines.
- Spend 50% of your time offering face-to-face supportive services in the field.
- Coordinate care with clinical teams and external agencies.
- Help consumers sustain wellness and recovery beyond program enrollment.
- Contribute to service improvements and consumer education.
- Ensure compliance with licensing and regulatory standards.
Job Requirements:
- Bachelor’s Degree in Social Work, Psychology, or related field.
- Experience with community-based services preferred.
- Strong computer skills (Word, Excel).
- Valid Driver’s License and acceptable driving record.
- Excellent communication, planning, and organizational skills.
