Reports To: Client Services Director, Grand Forks Housing Authority
SUMMARY OF POSITION
The Grand Forks Housing Authority is seeking a compassionate and dedicated Permanent Supportive Housing Case Manager to support individuals experiencing chronic homelessness in achieving housing stability, improving well-being, and accessing education or employment opportunities. This role will assist clients in navigating community resources and developing self-advocacy skills, while providing emotional support and advocacy on their behalf when needed. The position will be mobile, primarily serving clients in the greater Grand Forks area and Region IV of North Dakota.
ESSENTIAL FUNCTIONS AND DUTIES
- Build and maintain trusting relationships with individuals facing chronic homelessness, offering emotional support and consistent check-ins.
- Conduct person-centered planning with individuals, assisting them in identifying and achieving personal goals.
- Assess individuals’ current living conditions and discuss safety planning and harm reduction strategies.
- Guide clients through complex systems such as behavioral health treatment, healthcare, human services, affordable housing, SUD recovery, and the criminal justice system.
- Provide information, referrals, and support to help individuals access services they need.
- Use Motivational Interviewing techniques to help clients identify barriers to their goals and support creative problem-solving.
- Assist clients with housing identification, navigating the Coordinated Entry system, and addressing rental and financial needs.
- Maintain accurate and timely case notes and input data into the Homeless Management Information System (HMIS).
- Assist with preparing reports for HUD, grant renewals, and GFHA Executive Management.
- Stay informed on current trends and innovations in the homelessness, housing, and social services sectors.
REQUIRED QUALIFICATIONS
- Bachelor’s degree in Social Work or a related field required. MSW preferred.
- 1-2 years of experience providing direct services to individuals experiencing homelessness or navigating complex systems.
- Experience working with behavioral health conditions, substance use disorders, or individuals facing housing instability is a plus.
- Knowledge of housing assistance programs, behavioral health treatment, and social service systems.
SKILLS AND ABILITIES
- Proficient in Motivational Interviewing techniques.
- Ability to work independently in various community settings, including outreach in places not meant for human habitation.
- Minimum of 1 year of experience working with people facing behavioral health conditions and complex life challenges.
- Strong organizational and time-management skills.
- Ability to maintain professional boundaries and work collaboratively with minimal supervision.
- Valid driver’s license and automobile insurance required.
WORKING CONDITIONS
- This position involves both office and fieldwork, including outreach in challenging environments (e.g., outdoor settings and inclement weather).
- Travel required within Region IV and occasional national training opportunities.
- Ability to sit for prolonged periods and manage the physical demands of the role.
