General Description
The Case Manager at Families and Children Together (FACT) plays a pivotal role in supporting vulnerable families by coordinating and facilitating access to community resources and services. This position serves as a primary point of contact for families referred to FACT, working collaboratively with internal teams and external stakeholders to ensure comprehensive support and assistance.

Client Engagement and Assessment:
- Conduct initial assessments with families to identify their strengths, needs, and goals.
- Build rapport and establish trusting relationships with clients to facilitate effective service delivery.
- Maintain ongoing communication with clients to provide support and monitor progress.
Resource Coordination and Referral:
- Identify and connect families with community resources, including social services, healthcare, housing, and educational programs.
- Coordinate referrals to external service providers and agencies, ensuring timely access to needed support.
- Assist families in navigating complex systems and overcoming barriers to accessing services.
Case Management:
- Develop and maintain individualized service plans for each client, outlining goals, objectives, and action steps.
- Monitor and track client progress towards goals, providing guidance and support as needed.
- Document client interactions, assessments, and service referrals accurately and comprehensively.
Community Outreach and Engagement:
- Represent FACT at community events, meetings, and forums to promote awareness of services and resources available.
- Cultivate and maintain relationships with community partners, stakeholders, and service providers.
- Participate in collaborative initiatives aimed at addressing community needs and enhancing service delivery.
Advocacy and Empowerment:
- Advocate on behalf of clients to ensure their needs are addressed and their voices are heard within the community.
- Empower clients to advocate for themselves and navigate systems independently.
- Provide education and guidance to clients on rights, entitlements, and available resources.
Position Type:
Full-time, with occasional evening and weekend hours required for community events and client appointments.
Location:
The position will be based at FACT's Service Hub with frequent travel within the community to meet with clients and attend events.
