* Develops and implements individualized case plans, prioritizing permanency goals for each consumer. * Conducts regular home visits, case conferences, and other meetings to monitor progress and adjust service plans as needed. * Provides crisis intervention support to consumers and families during times of distress. * Collaborates with internal and external stakeholders to ensure coordination of services and resources.
Requirements
* Bachelor's degree in social work or related field. * Relevant experience in child welfare or a related field. * Excellent written and verbal communication skills. * Strong organizational and interpersonal skills. * Reliable transportation.
