Envision Hospice was established in 2005 by six healthcare professionals who wanted to make a difference in their community.
We are currently looking for a caring and hardworking Hospice Registered Nurse Case Manager (RNCM or RN)to join our growing team. This person will provide care to patients in various residential settings in the South King County, WA area (South of Seattle) including homes, assisted living facilities, and skilled nursing facilities.

Essential Functions:
- Provides Hospice nursing services in a variety of settings.
- Initiates nursing procedures appropriate for the patient’s Hospice care and safety.
- Observes signs and symptoms and reports to the physician and Interdisciplinary Group members any unexpected changes in the patient’s physical or emotional condition.
- Teaches and gently guides the Hospice patient and family members about providing care for the patient.
- Supervises and trains other nursing service personnel.
- Develops and re-evaluates the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care.
- Performs specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctor’s orders.
- Documents timely and follows all policies, procedures, and compliance requirements as required by Envision.
- Participates in the on-call rotation.
Qualifications:
- Graduate from an accredited school of nursing - RN - Registered Nurse
- Currently licensed as a registered nurse to practice in the state.
- Must have a CPR certification and/or must be up to date before start date.
- Experience working for a Hospice Agency as a nurse is preferred.
- Experience as a Case Manager is preferred, can be in another setting.
- Minimum 2 years of experience as a registered nurse preferred, at least 1 year is required.
- Must provide evidence of annual TB tests and other state-required tests or examinations.
- Must be able to work independently, make judgments based on assessments and data available, and act accordingly.
- Must have a current valid driver’s license, auto liability insurance, and reliable transportation.
